STJ CES Access Point - Carrie Part 1 Entry (Homeless family w/ referrals)
Greetings,
STJ CoC Coordinated Entry Access Points can assess clients who are at risk of homelessness for Prevention/Diversion resources, and clients who are literally homeless and need to be considered for placement on the PL by the List Holder. This training module will cover how to enter data for a household that is literally homeless, so the enrollment and referral process for placement on the Prioritization List can be reviewed.
All applicable steps of this practice case must be completed satisfactorily by [DATE] at 5 pm or your training will be closed, and a designated authorized representative of your agency will need to re-submit the required forms for you to be trained, at which point you will be required to start over from the beginning.
Before entering your practice case, you will need to set up some Counts Reports on the Home Page Dashboard to become familiar with that process. Counts Reports will help you keep track of clients enrolled in your STJ CES provider, and referrals made to the List Holder for PL placement review.

To ensure you know how to manage clients and referrals in Community Services for the STJ CES, please use this guide to set up the four Counts Reports identified on Home Page Dashboard. Be sure to save your changes when you are done.
