SSVF RRH - Samuel Scott (family), part 1

The first part of your practice case is to enter the client "Samuel Scott," his spouse, and their child into the training site. To assist you in doing so, we've developed the following training materials and guides:

  • Training Video: SSVF Entry Basics (password: CommunityServices)

    This training video will walk you through the steps of putting a family into SSVF Rapid Rehousing. The video is 23 minutes long. You can watch it as many times as you need to.

  • Workflow: Client Workflow for SSVF Projects (pdf)   

    This workflow guides you through the steps of entering, updating, and exiting clients from SSVF funded projects. We strongly encourage you to utilize this document while entering your practice case, and that you keep it handy while you finish the entire practice case and while entering real clients into the live site.

  • SSVF Specific Service Transaction Records: SSVF Crosswalk (pdf)

    This document identifies the associated HMIS service term/code for each SSVF service/financial assistance type to be used when recording SSVF specific service transaction records in Community Services.

  • Practice Case: Samuel Scott's family, part 1 (pdf)    
  • This form should be treated as if it were an intake form. Utilize the information on the form to create the client records and to enter the clients into the project.
