Full SPDAT in WellSky Community Services

This guide instructs HMIS users with access to the Full SPDAT in WellSky Community Services on the steps required to input the initial and follow-up full SPDATs into WellSky Community Services, as well as how to print the completed SPDATs when needed. 

Reminder: Access to the full SPDAT within WellSky Community Services is limited only to users within CoCs which have been trained by OrgCode or an OrgCode authorized trainer to utilize the full SPDAT. Questions may be directed to our helpdesk.

Recording the Initial SPDAT

Before starting, make sure you've set your Enter Data As appropriately, you've opened the client record in the Clients module, then set your back date to the date the initial full SPDAT was completed.

Go to the Measurements tab and click the Add New Measurement button.

Select the appropriate measurement tool, verify that the date is correct, and record the correct user or individual who administered the full SPDAT. Click Continue, and the summary version of the questionnaire will load.

Click the magnifying glass in front of the first domain. Using the prompts and score information displayed, choose the appropriate score. In the text box at the bottom of the screen, add comments explaining the chosen score. Click Submit & Move to Next.

Repeat step 3 until all domains have been completed. On the final domain, click Submit & Close.

Verify that all rows have been completed, then click Save & Exit.

Recording Follow-up SPDATs

Before starting, make sure you've set your Enter Data As appropriately, you've opened the client record, then set your back date to the date the follow-up full SPDAT was completed.

Go to the Measurements tab, and click the Add New Point of Measurement button.

Select the appropriate Point of Measurement*, verify that the date is correct, and record the correct user or individual who administered the full SPDAT. Click Continue, and the summary version of the questionnaire will load. 
*Note: Most points of measurement can be used only once, except for "Other" and "At Move." After 1 year, you will need to use only one or both of these two options.

Click the magnifying glass in front of the first domain. Using the prompts and score information displayed, update the appropriate score. In the text box at the bottom of the screen, add/edit comments explaining the chosen score. Click Submit & Move to Next.

Repeat step 3 until all domains have been completed. On the final domain, click Submit & Close. On the final domain, click Submit & Close.

Verify that all rows have been completed, then click Save & Exit.

Printing Completed SPDATs

Use the pencil or magnifying glass to open the SPDAT you want to print out.

Click the printer icon in the upper right corner of the measurement popup, then check/uncheck options to select which items you want printed:
Checkbox Description
Include Domain Scores If checked, the score (4, 3, 2, 1, 0 or N/A) for each domain will be included in the printout.
Include Comments If checked, the comments for each domain will be included in the printout.
Include Domain Descriptions Table If checked, the descriptions and scoring criteria for all domains will be included in the printout.
Include Score Range Descriptions Table If checked, the definitions of the final scoring ranges (no housing intervention, rapid rehousing, permanent supportive housing) will be included in the printout.

Click Print, then follow printer prompts (varies by computer and printer).
