PATH - Peter, part 4 (with services)

Now that you've successfully finished the third part of your practice case, the fourth and final part is to exit "Peter" from your project on the training site. To assist you in doing so, we've developed the following training materials and guides:

  Please go through this interactive training (password: CommunityServices). This interactive training goes at your own pace but usually takes less than 20 minutes. You can watch it as many times as you need to.
• Practice Case:  Peter, Part 4 - Exit (pdf)
  Utilize the information on this form to complete the exit assessment.
