Entry/Exit Type
Every provider (project) in WellSky Community Services is assigned an Entry/Exit Type when set up in the system. The Entry/Exit Type is associated with a specific assessment or group of assessments based on the funding source and/or type of project for that provider. Selecting the correct Entry/Exit Type ensures all the fields for the required data elements are available for information to be recorded. When enrolling a client into a provider, you will find you are prompted to select the Entry/Exit Type on the Entry Data screen, as shown below.
As you can see, there are several options to choose from for Entry/Exit Type, so it imperative to select the correct type for each provider to ensure the required data is captured and funder reports run properly. The guidance below outlines the criteria to consider when selecting the Entry/Exit Type.
Basic – Currently not used for any provider!
HUD – Used by CoC and ESG funded providers, and other funding sources as applicable. The assessments associated with this Entry/Exit Type for CoC and ESG providers include: HUD CoC/ESG Entry, Update and Exit assessments; and the Special Needs Assessment. All other providers will use the Core Data Elements Entry, Update and Exit assessments.
PATH – Used by PATH funded providers only. The assessments associated with this Entry/Exit Type include: HHS PATH Entry, Update and Exit assessments; and the Special Needs assessment.
Quick Call – Currently not used for any provider!
RHY – Used by RHY funded providers only. The assessments associated with this Entry/Exit Type include: HHS RHY Entry, Update and Exit assessments; and the Special Needs assessment.
Standard – Used by MHTF/MoHIP providers funded through MHDC only, non-mandated providers (as applicable), and Coordinated Entry providers (as applicable). The assessments associated with this Entry/Exit Type for MHTF/MoHIP providers include: MHDC MHTF/MoHIP Entry, Update and Exit assessments. Locally funded providers will use the Core Data Elements Entry, Update and Exit assessments. Coordinated Entry providers will have customized assessments.
VA – Used by SSVF, GPD, and HCHV funded providers. The assessments associated with this Entry/Exit Type for SSVF providers include: VA SSVF Entry, Update and Exit assessments. GPD and HCHV providers will use the Core Data Elements Entry, Update and Exit assessments; and the Veteran Information assessment.
If the correct Entry/Exit Type is selected, you will find the corresponding assessments in the Entry Assessment section, as shown below.
If the wrong Entry/Exit Type is selected, you should see a message in the Entry Assessment section that no assessment has been specified for this provider, as shown below.
