Creating a Goal

Reminder: Goals are shared with other agencies in HMIS, except goals created by protected providers. Case notes entered within the goals are not shared with other agencies, ever.

  1. Select appropriate Enter Data As provider
  2. Search for the client and select the record
  3. Set the appropriate back date
  4. Click on the Case Plans tab
  5. Click the Add Goal button
    1. If a household exists, select all family members to whom the goal will apply
  6. Select Provider by making a selection from the drop down menu
  7. Type in the Date Goal was Set
  8. Set Classification by making a selection from the drop down menu
  9. Set Type by making a selection from the drop down menu
  10. Set Overall Status to Identified
  11. Click the Add Goal button
  12. Adding a Case Note
    1. Click the Add Case Note button
    2. Type the case note
    3.  Click the Save Case Note button
  13. Click the Save & Exit button

Updating a Goal

Reminder: Goals are shared with other agencies in HMIS, except goals created by protected providers. Case notes entered within the goals are not shared with other agencies, ever.

  1. Select appropriate Enter Data As provider
  2. Search for the client and select the record
  3. Set the appropriate back date
  4. Click on the Case Plans tab
  5. Click on the Notepad Icon next to the goal being updated
  6. Click on the Add Case Note button
  7. Select Provider by making a selection from the drop down menu
  8. Type the case note
  9. Click the Save Case Note button
  10. Click the X at the top right corner to close the pop-up box

Closing a Goal

Reminder: Goals are shared with other agencies in HMIS, except goals created by protected providers. Case notes entered within the goals are not shared with other agencies, ever.

  1. Select appropriate Enter Data As provider
  2. Search for the client and select the record
  3. Set the appropriate back date
  4. Click on the Case Plans tab
  5. Click the pencil in front of the goal to be closed
    1. If a household exists, select all family members which the goal will apply
  6. Set Overall Status to Closed
  7. Set If Closed, Outcome by making a selection from the drop down menu
  8. Type in the date of closure (to the right of If Closed, Outcome)
  9.  Adding a Case Note (if applicable)
    1. Click the Add Case Note button
    2. Type the case note
    3. Click the Save Case Note button
  10. Click the Save & Exit button
