Anniversary Date and Window Report

This guide is provided to assist users in identifying a household's anniversary date and the permitted anniversary window for the household according to HUD's HMIS Data Standards. 

Important! This report does NOT identify whether a client has an annual assessment that meets the requirements established for a particular grant period. ICA recommends using table 6c of the CoC-APR or ESG-CAPER to identify whether there are any clients in the project with annual assessment errors.

Setting the Prompts

Prompt Instructions
Select Provider(s): Select the project(s) for which you wish to run the report. 
EDA Provider If only one project is selected in the "Select Provider(s):" prompt, select the same project here.  If more than one project is selected in the "Select Provider(s):" prompt, then select one of the projects within the selected projects or reporting group. 
Enter effective date Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021.

Using the report

This report pulls in all active heads of household, as well as any clients who are in households without a head of household, as of the effective date of the report. The report lists clients in order based upon their anniversary date. It is important to remember that, per the HUD HMIS Data Standards, the annual assessment must occur within the anniversary window around the head of household's anniversary date. 

In the event a household is showing with either more than one head of household or less than one head of household, the entire household will be listed and the rows for each household member will be highlighted in yellow. This is to assist you in identifying possible errors around the household composition. 

Important! Because the report contains identifying information (names), the report cannot be emailed (even within agencies) unless encrypted, nor can it be shared outside of the agency.

Report columns

This information is provided to assist with interpreting the information in the report.

Anniversary Date

This is the date of the client's project start anniversary each year. 

Client ID

This is the client's ServicePoint ID number.

Client Name

This is the client's name in "Last, First" format. 

Entry/Exit Provider

This indicates the provider under which the "Entry/Exit" is recorded.

Anniversary Window

Using the anniversary date, this shows the appropriate anniversary window. 

Note: This anniversary window column on this report does not take into consideration the grant cycle of the project. If a client's anniversary window falls around the start date or end date of the grant cycle, the anniversary window will be reduced to fall within the grant period. Here are two examples:

Ex. 1: The grant cycle runs 10/1 to 9/30, and the client's project start date is 10/5. The start date for the anniversary window for the client is actually 10/1.

Ex. 2: The grant cycle runs 7/1 to 6/30, and the client's project start date is 6/20. The last date for the anniversary window for the client is actually 6/30.

HoH Errors

This column will show one of three things:

Symbol/Message Meaning
- There is no head of household error in this record. No action is necessary.
No HoH There is no head of household identified in this household group. Make adjustments so that one household member is identified as the head of household.
Multi HoH There is more than one head of household identified in this household group. Make adjustments so that only one household member is identified as the head of household.
