PATH Service Transactions

This guide is designed to provide guidance to PATH HMIS users in properly recording PATH-funded Services. 

PATH Service Transactions

The HMIS data element P1 Services Provided–PATH Funded is used to determine the PATH-funded services that are provided to a client during project enrollment, following project enrollment and prior to project exit. The PATH Annual Report only requires that you collect one response for each service provided[.] ... PATH providers are not required but encouraged to record each instance of PATH-funded services provided to PATH-enrolled clients[.]

PATH Service Transactions must be recorded once the client is PATH-enrolled. If the client is not PATH-enrolled, no services need to be documented. Any services recorded for clients who are not PATH-enrolled or recorded prior to the Date of PATH Status Determination (if enrolled) will not be included in the PATH Annual Report. 

Steps to Record PATH Service Transaction

  1. Make sure you've set your Enter Data As and your back date. The client's entry into the project should have been recorded in the HMIS prior to recording any service transactions, and the client must be PATH-enrolled for the services to be included in the PATH Annual Report.
  2. Within the client record, click on Service Transactions.
  3. On the Service Transaction Dashboard, click Add Service.

  4. Ensure that the correct Service Provider is displayed and that your Start Date and End Date are correct. The start and end date fields should match the day the client was provided this particular service. 

  5. Select the correct Service Type from the dropdown menu. Refer to the "HMIS Service Description" column in the PATH Service Crosswalk below to determine the appropriate type of service to select. Leave the Provider Specific Service dropdown as -Select-. 
  6. Click Save & Continue.
  7. Select the appropriate option in the Type of PATH FUNDED Service Provided dropdown

  8. There is no need to record any items in the Service Costs section, to Apply Funds for Service, to upload Support Documentation, or complete Follow-Up Information
  9. Scroll down to Need Information and update the Need Status and Outcome of Need according to the client's situation.

  10. Click Save & Exit.
