Definitions
Client Coordinated Entry Status
At the time of assessment/re-assessment for coordinated entry, the client/household was...
Homeless - Place Not Meant for Habitation
Living in a place not meant for habitation (e.g., street/sidewalk, bus station shelter, tent, vehicle).
Homeless - Emergency Shelter
Living in a shelter (including congregate shelters, hotels & motels paid for by charitable organizations or government entities).
Homeless - Transitional Housing
Living in a transitional housing program designed provide homeless individuals with temporary living arrangements.
Homeless - HUD Category 3 (Youth Only)
Couch surfing or otherwise doubled up and all household members are 24 and under.
Homeless - HUD Category 4
Fleeing/attempting to flee domestic violence, dating violence, sexual assault, stalking or other dangerous or life-threatening conditions that relate to violence against the individual or a family member, including a child, that has either taken place within the individual's or family's primary nighttime residence.
Homeless - Institutional Stay (Under 90 Days)
Currently in an institution for less than 90 days, was homeless prior to entering the institution, and is expected to return to homelessness after leaving the institution.
Transferring (Lateral Move)
Living in a housing placement but have been approved by the Acuity Review Panel to be transferred to another housing placement when an applicable opening is found.
Housed
Living in a permanent residence. Anyone with this status must be exited as they are not eligible to be on the prioritization list.
Coordinated Entry Referral Outcomes
The referral outcome for any housing placement referrals must be kept up to date using the following definitions:

The agency has not yet scheduled the eligibility interview.
Accepted
The client has been housed by the project.
Accepted on Wait List
Clients should have this status from the point that the housing provider has scheduled an eligibility interview with the client to the point they are housed or the referral is canceled or declined.
Canceled
Cancelations are on the part of the client. The housing provider has contacted the client and has determined that the client does not want or need housing assistance from their agency, OR the agency has made multiple attempts to contact the client and has been unable to do so. Reason for cancelation is recorded within the HMIS. The housing provider must notify the front door lead.
Declined
Declinations are on the part of the agency. The housing provider has determined that they cannot serve this particular client. The reason for the declined referral is recorded within the HMIS. The housing provider must notify the front door lead.
