AHTF Data Collection Forms
ICA has developed these data collection forms to assist with the collection of data for projects that are funded by the Affordable Housing Trust Fund (AHTF) of the City of St. Louis. The forms are laid out so that they are consistent with the data entry workflow within the HMIS. All forms are available in both Word Document (docx) format and PDF format. Use of these forms is optional.
