St. Louis CITY C.E.: Exiting a Client/Household from Coordinated Entry

This guide provides the basic steps for removing a client/household from the prioritization list for St. Louis City CoC. For additional assistance beyond this guide, please contact the helpdesk.

Please keep in mind that any questions relating to St. Louis City Coordinated Entry that are not specifically tied to HMIS or to the Non-HMIS Prioritization list should be directed to the St. Louis City CoC's Coordinated Entry Committee. 

Home Page Dashboard

  1. Click Enter Data As in the upper right-hand corner. 
  2. Click the Green Plus Icon to the left of CITY St. Louis - CITY Coordinated Entry to set your EDA to the coordinated entry project.

Clients

  1. Enter the primary client's First and Last Name and click Search. Click on the pencil icon to the left of the client name to open the client record (ensure name, DOB, and SSN all match).

Client Profile tab

  1. At the back date prompt, change the date to the correct back date (see notes below), then click Set New Back Date
    • For housed clients, the back date must match the date they moved into their housing unit.
    • For clients who request to be removed, the back date must match the date they requested to be removed from the list. 
    • For clients who are no longer in contact, the back date must match the last date of contact. 
    • For clients who have an expired CE Participation Agreement, it must match the date the CE Participation Agreement expired unless their last date of contact was before this date, in which case it must match the last date of contact.

Entry/Exit tab

  1. Click the exit pencil icon on the open exit assessment for the CITY St. Louis - CITY Coordinated Entry project.  
  2. Confirm/set Exit Date
    1. Ensure the Exit Date matches the appropriate exit date using the method described in the section above. 
    2. Set the Reason for Leaving according to the situation:
      • For housed clients, whether housed thru Coordinated Entry or not, select: Completed Program.
      • For clients who request to be removed, select the option that best fits the reason the client requested to be removed.
      • For clients who are no longer in contact, select Unknown/Disappeared
    3. Set the Destination according to the situation:
      1. Becoming housed through Coordinated Entry
        1. Permanent Supportive Housing (incl. Shelter Plus Care): Permanent housing (other than RRH) for formerly homeless persons
        2. Rapid Rehousing: Rental by client, with RRH or equivalent subsidy
        3. Transitional Housing: Transitional housing for homeless persons (including homeless youth)
      2. Client requested removal: [Select the option which most closely matches where the client anticipates spending the night.]
      3. Becoming housed outside of Coordinated Entry: [Select the option which appears to be most appropriate.]
      4. Client disappears: No exit interview completed
    4. Click Save & Continue
  3. The exit assessment will appear. Complete/update fields as appropriate:
    1. ICA CITY St. Louis C.E. - Project Exit
      This assessment is filled out for the Head of Household and all adult clients (18+). Enter any updated information as applicable.
      Note: If your housing project does not appear in the dropdown, please contact the helpdesk so that we can assist you by adding the project. 
  4. Click Save & Exit once all applicable fields have been completed.
