BoS Coordinated Entry: Adding and Removing Clients to/from the Prioritization List
Clients are now added to and removed from the BoS CE PL by completing the BoS CES Prioritization List ADD/REMOVE sub-assessment. This article will review how to properly complete both of those processes.
Adding a Client to the PL
The top section of the BoS CES Prioritization List ADD/REMOVE sub-assessment in the BoS CoC CES Entry Assessment must be completed for the HoH only.
The Prioritization List Start Date will auto-populate to the Back Date set as the entry date, so check to make sure it is correct. Select your agency's BoS CoC CES project from the Provider adding client to the Prioritization List dropdown menu, then click Save. This client is now on the active PL. Do not enter any information in the bottom section of this sub-assessment at this time.
Exiting a Client who is Working with Another Agency
If you are exiting a client from your BoS CoC CES provider because they are now working with another agency, do NOT remove them from the PL. Instead, use Option 1 in the BoS CoC CES Exit Assessment to indicate whether they are working with another access point in your services area or with another access point in a different service area.
Removing a Client from the Prioritization List
If you are exiting a client from your BoS CoC CES provider because they have been housed or because they are inactive, use Option 2 in the BoS CoC CES Exit Assessment and complete the bottom section of the BoS CES Prioritization List ADD/REMOVE sub-assessment that placed the client on the PL originally. Be sure to click on the pencil icon to the left of the original sub-assesment that placed the client on the PL in order to edit the previously recorded information. Do NOT click Add.
Do not change any of the information in the top section of this sub-assessment. Complete the bottom section only to remove the client from the PL. Enter the Prioritization List End Date, select your provider from the Provider removing client from the Prioritization List dropdown menu, and select the appropriate Exit Outcome. If the client was housed, indicate how housing was achieved and county where now housed. The Housing Move-In Date is entered by your BoS CoC CES provider only when the client self-resolved to housing, or when the client has been housed by a non-HMIS provider, such as an Emergency Housing Voucher (EHV). Otherwise, it is entered by the housing provider when the client was actually housed, so do not enter or edit this date under those circumstances. If the Housing Move-In Date is blank, but shouldn't be, reach out to the housing provider directly to have that date entered by them, or discuss this during your next case conferencing meeting as something that needs to be addressed. If the Prioritization List End Date and Housing Move-In Date do not match, answer Reason dates do not match. Click Save.
Remember, it is the responsibility of each Access Point to remove their own clients from the BoS CE PL in a timely manner, so the active list accurately reflects current clients.
