STJ CES List Holder - Processing Housing Related CE Referrals

STJ CES Access Points will enter a Housing Related Coordinated Entry referral for all homeless clients, so the List Holder can review the client’s record and determine if they are eligible to be placed on the PL. The List Holder will follow the steps outlined below to process those referrals.

From the Home Page Dashboard, click on the blue number in the Outstanding Incoming Referrals Counts Report, then click the blue Client ID to access the client’s record.

Set the Back Date to the day the client record was reviewed for PL placement, then navigate to the ROI tab and click Add Release of Information. Check all household members and complete the remaining fields as indicated.

            Provider: Verify this is “Interserv – STJ CES List Holder”.

            Release Granted: “Yes”.

            Start Date: Verify it is correct.

            End Date: Enter a date that is one year from the Start Date.

            Documentation: “Verification from Other Institution”.

            Witness: Enter the name of the referring Access Point.

Go to the Entry/Exit tab and enroll the household into your List Holder project.

Review the Housing Needs Assessment for completeness, and determine if client will be placed on the PL. If the client will be placed on the PL, complete the STJ CES Prioritization List ADD/REMOVE sub-assessment, and add any needed notes.

If the client will not be placed on the PL, go ahead and exit the client/household from your STJ CES List Holder provider, select Not Placed on PL as the Exit Outcome, then proceed to the next step to close the referral. However, if the client is placed on the PL, do not exit the client/household at this time. The exit will be completed when the HoH needs to be removed from the PL, so skip this step and close the referral. Side note: You may elect to assign yourself as the HoH’s case manager on the Case Manager tab, so the “My” Counts Reports can be used for a list of clients in your project comparable to the PL.

Next, you will need to close this referral by going to the Service Transactions tab and clicking on the View Entire Service History tile.

Locate the appropriate referral in the list of All Service Transactions, and click on the pencil icon to the left of the referral.

Scroll to the Need Status and Outcome section and follow the appropriate process below.

If the client was placed on the PL, change Need Status to Closed and Outcome of Need to Fully Met. Click Save & Exit.

If the client was not placed on the PL, change the Need Status to Closed, Outcome of Need to Not Met, and Reason to Client Not Eligible. Click Save & Exit.
