BoS Prevention Prioritization Report

This report includes information from the Crisis Needs Assessment for clients identified as Prevention only as a result of completing the Crisis Needs Assessment. Please note that agencies must establish their own standards for prioritizing prevention clients, so unlike the BoS Prioritization List Report, clients in this report are not listed in order of priority.

Setting the Prompts

Prompt Instructions
Select Provider(s): Select the project(s) for which you wish to run the report. 

Leave blank if using the "Select All Counties in Region:" prompt.
EDA Provider If only one project is selected in the "Select Provider(s):" prompt above, select the same project here. 

If more than one project is selected in the "Select Provider(s):" prompt, then select one of the projects. 
Enter effective date Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021.
Enter Report Start Date: Enter the start date for the reporting period in m/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2020.
Enter Report End Date: Enter the date after the last date of the reporting period in m/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021.
Select All Counties in Region: Use this prompt to pull all clients assessed in a specific county/region.  Only counties associated with a P/D Assessment will display in the Select All Counties in Region prompt. Use the Search Field and enter "Region 1", "Region 2", etc. to quickly pull all counties in a region.

Leave blank if you are running this report by using the "Select Provider(s):" prompt.

Using the Report

The BoS Prevention Prioritization Report pulls responses in the Crisis Needs Assessment relevant to prioritization, including scores. This allows agencies to determine the information used to establish their own prioritization guidelines.  Please see below for further details about selected columns on the BoS Prevention Prioritization Report.

Note that it is essential to ensure that all information on the Crisis Needs Assessment has been filled out completely and accurately in order for this report to pull the appropriate clients and return correct prioritization information. Only the most current information for each field in the Crisis Needs Assessment will be pulled into the report, so it is important to update fields, as needed when clients are assessed.

Crisis Needs Assessment Date

Clients will only be included in the BoS Prevention Prioritization Report if the "Date assessment completed" falls between the Report Start Date and the Report End Date. It is important to ensure that this date is filled out and that the correct date has been listed.

Crisis Needs Assessment Provider

If the "Select Provider(s):" prompt is used to run the BoS Prevention Prioritization Report, then this column will only list the Provider(s) indicated by that prompt. If the report is run using the "Select All Counties in Region:"prompt, then any Provider that completed a Crisis Needs Assessment with a client in one of the counties selected will appear in this column. Please note that this field should be updated when a new BoS CE Provider completes a Crisis Needs Assessment with the client. 

Crisis Needs Assessment County

If the  "Select All Counties in Region:" prompt is used to run the BoS Prevention Prioritization Report, then this column will only list clients who reside in the county or counties indicated by that prompt. If the report is run using the  "Select Provider(s):" prompt, then this column will include any client that completed a Crisis Needs Assessment in the given date range and their county of residence.  It is important to ensure that the county field is filled out when completing a Crisis Needs Assessment and that the correct county has been listed.

Where Did You Stay Last Night? 

Clients will only be included in the BoS Prevention Prioritization Report if the answer to "Where did you stay last night?" is one of the following:

  • With friends/family
  • Hotel or motel paid for by self/friend/family
  • In my own housing
Night Prior Score

Only clients who have been staying with friends/family, in a hotel or motel paid for by self/friend/family, or in their own housing are pulled into the report. As such, this score should be a "1" for all clients.

Income Score

Scores in this column should range between 0 and 3 according to the following criteria: 

  • AMI is greater than 50%: 0
  • AMI is below 50%: 1
  • AMI is below 30%: 2
  • AMI is below 25%: 3
Ratio Score

This field captures information about the "Housing Income to Expense Ratio". If the client does not pay rent or if their income to expense ratio is less than 40%, then this field should be 0. If the income to expense ratio is greater than 40%, a score of 1 should be assigned.

Notice Score

This field should have a score of 1 if the client has an eviction notice and a score of 0 if the client does not.

Moved Score

This field should have a score of 1 if the client has moved two or more times in the last year and a score of 0 if they have moved once or not at all in the last year.

Resided Score

This field should have a score of 1 if any household member has resided in an Emergency Shelter or in a place not meant for human habitation within the last five years and a score of 0 if they have not.

Assistance Score

This field should have a score of 1 if the client is receiving any type of ongoing rental assistance and a 0 if they do not.

Prevention/Diversion Assessment Score

This column should provide a total score that is the sum of the Night Prior Score, Income Score, Ratio Score, Notice Score, Moved Score, Resided Score, and Assistance Score.
