Project Entries through BoS Coordinated Entry Report (Side Door Report)

This report includes information about whether clients enrolled in HMIS-participating Rapid ReHousing (RRH) and Permanent Supportive Housing (PSH) projects in the Balance of State were referred to that project through Coordinated Entry. Many funders require that clients be referred to RRH and PSH projects through Coordinated Entry, and this report allows agencies to identify any clients who do not have a referral from Coordinated Entry recorded in HMIS. If a client was referred to the project through Coordinated Entry but does not have a referral recorded in HMIS, this report gives agencies an opportunity to follow up with their regional list holder to ensure that a referral is properly recorded.

Enter effective date Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021.
Enter Start Date: Enter the start date for the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2020.
Enter End Date PLUS 1 Day: Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021.

Using the Report

This report has three tabs, each serving a specific purpose. 

Summary Tab

This tab includes a complete list of all HMIS-participating Rapid ReHousing and Permanent Supportive Housing projects. Only the projects that appear on this tab in bold are required to participate in Coordinated Entry by their funder. This tab includes columns indicating the Project Name, the total Number of Entries into the project during the reporting period, the total Number of Entries with a CE Referral, and the Percentage of Entries with a CE Referral. For projects required to participate in Coordinated Entry, the Percentage of Entries with a CE Referral should be 100%.

Client Detail Tab

This tab provides a breakdown of all clients enrolled into HMIS-participating Rapid ReHousing and Permanent Supportive Housing projects during the reporting period. If your project is required to participate in Coordinated Entry, you will use the Client has CE Referral? column to identify any clients who do not have a CE Referral. If you determine that any clients who do not have a CE Referral recorded in HMIS actually were referred to the project through Coordinated Entry, please reach out to your regional list holder to ask them to record this referral in HMIS.

This tab contains information about the report that can be utilized by ICA staff to determine whether the report was run properly by the user.
