St. Louis City C.E. - Deep Dive Assessment Tip Sheet

This assessment is for those clients who have consented to participate in the coordinated entry process. The Deep Dive assessment is used to collect and store all the data necessary for a successful coordinated entry housing placement through the weekly housing priority list (WHPL) process. Questions cover case manager housing recommendations with an explanation area and drop-down menu for the appropriate housing type, current homelessness documentation, disability documentation, length of time homeless and any forms of identifications needed. Any additional notes can be placed in the text box section. 

This assessment must be answered if the client has been placed in the Priority Pool. All questions now REQUIRE an answer before you can save the assessment. 

Filling Out the Form

Set your EDA to the correct project for your Coordinated Entry project. THIS IS VERY IMPORTANT.

Click on the Clients Tab and go to the appropriate client profile.

The Deep Dive assessment is accessed, either as one of the assessments at the time of enrollment in the C.E project OR at a later time, by clicking on the Interim Update option to add the information at that time. You will choose the  ICA City St. Louis C.E. - Deep Dive from the assessments and complete the information needed. Anytime you need to add new or change information collected, you will do another Interim update so that the historical information is kept intact.

 The questions should be answered for Head of Household. (Note that uploading of documentation to file attachments on the client profile tab is encouraged so they are already in HMIS if needed)

Once completed, scroll to the bottom and click Save.   (This information can be printed for a copy to maintain in the client's file if desired.)
