St. Louis City C.E. - Project to Project Transfer Form

his is an assessment form that will be utilized any time there is a need in Permanent Housing to make a client transfer from one agency to another. This form can be accessed by any users with C.E. project access. The assessment will store all the necessary information such as reason for transfer, all current services, disability documentation, homelessness verification and other housing needs for successful transfer. There will be a Project-to-Project Transfer Application Report pulled weekly from this assessment, by the City St Louis C.E. Manager. This form can be printed out as needed. Form

Filling Out the Form

Set your EDA to the correct C.E. project and find the appropriate Client Profile. (Note, this assessment can only be seen once in the correct EDA)

Click on the Client Profile Tab and select the  ICA CITY St. Louis C.E. - Project to Project Transfer towards the bottom of the page.

    Click "Add" to begin a new form. 

Note that you  must upload all documentation of disability and chronic homelessness into HMIS prior to the transfer being considered.

 When the questions appear, please answer all of them completely. 

Once completed, scroll to the bottom and click Save.
