This guide is provided to assist users in understanding the Graphic and table summary of YHDP project level data, including client detail report with YHDP - specific error alerts.

Setting the Prompts:


Prompt Instructions
Select Provider(s): Select the project(s) for which you wish to run the report.
EDA Provider If only one project is selected in "Select Project(s):" above, select the same project here.   If more than one project is selected in "Select Project(s):"
Enter End Date PLUS 1 Day: Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2023-6/30/2024, the date entered in this box would be 7/1/2024.
Enter Start Date: Enter the start date for the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2023-6/30/2024, the date entered in this box would be 7/1/2023.
Enter effective date Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2023-6/30/2024, the date entered in this box would be 7/1/2024.

Using the Report

This report has four tabs:

  • All Projects Graphic Summary: Readily available analysis of data for single projects or multiple projects grouped. Details below.
  • All Projects Summary Tables: Granular summary data including race, gender, household type and health scores.
  • Client Detail: Row data for each client enrollment, including all YHDP specific data elements. Cells that are highlighted flag potential data quality problems.
  • Additional Information: This tab contains information about the report that can be utilized by ICA Reporting Team to determine whether the report was run properly by the user. If you seek assistance with this report from ICA Reporting Team, please be prepared to read/send information off of this tab to the individual who assists you. 

Guide to YHDP Graphic Summary

Chart Description

All clients (current and exited)

All clients. Count of enrollments may be higher than count of clients where clients are enrolled in multiple programs included in report.

Current clients

Clients with an entry date and no exit date in one or more YHDP project. All other dashboard counters include exited clients.

Adults no children

Total number of clients age>=18.

Minor heads of household

Clients under age 18 who are unaccompanied by adults or parents.

People in households with children

All people in families.

Children of parenting youth

All clients where "Head of household's child" is selected.

Currently fleeing domestic violence

All clients who answer "Yes" to "Are you currently fleeing?" after answering yes to "Survivor of Domestic Violence".

Pregnant

All clients who answer yes to the "Pregnancy" question.

Moved in with RRH

All Rapid Rehousing clients with a Housing Move in Date >= Entry date and<= Exit Date.

Permanent exits

All clients with permanent exit destinations. (For details about which exit destinations quality as permanent exits, see the 2024 HMIS Data Standards, Appendix A: https://files.hudexchange.info/resources/documents/HMIS-Data-Standards-Manual-2024.pdf)

Type of Homelessness Housing Instability

Analysis of Prior Living Situation data according to HUD Data standards and definitions. "At Risk" is a catch all category for all clients who are not Category 1 (literally homeless) or Category 4 (fleeing domestic violence). Imminent risk for homelessness can be difficult to validate in HMIS data.  Heads of household and adults only.

Proportion BIPOC

White = White only, not Hispanic or Latino. BIPOC=All other race identities and race/ethnicity combinations.

Age Distribution

Each bar represents the number of clients of that age.  While YHDP project enrollments for those who are 25 and older may indicate data entry errors, please note that there may be legitimate circumstances where individuals older than 24 are enrolled in YHDP projects. These may be clients who were enrolled in the project prior to turning 25 who are still eligible to receive YHPD services, or these may be adults who joined the household after a youth Head of Household's was already enrolled in the project (please note that these household members do not qualify the household for any additional assistance). 

General Health

Responses to General Health Status Question. Heads of household and adults only.

School Enrollment / Attendance

Responses to education status question. Heads of household and adults only.

Proportion LGBTQ+

"LGBTQ+" includes all clients who report any sexual identity other than "Heterosexual" or any transgender experience. "Cis Het" includes clients who select "Heterosexual" and either "Man (boy if child)" or "Woman (girl if child)" with no accompanying data suggesting transgender experience. Missing Sexual Orientation data accounts for most clients in "no data" category. Heads of household and adults only.

Formerly a Ward of the Juvenile Justice System

Clients who responded "Yes" for "formerly a ward of the juvenile justice system."  Heads of household and adults only.

Formerly a Ward of Child Welfare / Foster Care

Clients who responded "Yes" for "formerly a ward of child welfare / foster care." Heads of household and adults only.

Education

Separate charts for minors and youth over age 18 show combined results from three education questions on enrollment, attendance and highest level achieved. Heads of household only

Days from Approximate Date This Episode of Homelessness Started

Shows self-reported pre-enrollment time homeless in ten, then 100 day units.  Heads of household and adults only.

Days Enrolled in YHDP Project

Days from project start date to project exit date if exited. Days from project start date to report end date for current clients, in ten, then 100-day, units.

Outcome Categories 

Bar chart detailing how exit destination correlate to positive outcome analysis, according to HUD data standards. For unsheltered Street Outreach clients, temporary destination are positive. Current enrollments are included for context. See Outcomes Table for more information.

Shows proportion of BIPOC clients according to categories of homelessness.

Shows proportion of LGBTQ+ clients according to categories of homelessness.

Safe Havens are funded by HUD CoC Safe Haven grants, and are specifically designed to serve hard to reach people experiencing homelessness with severe mental illness who have been unsheltered and have been unwilling or unable to participate in supportive services.

Sometimes the term "Safe Haven" is used in reference to a Victim Services Provider or Emergency Shelter - however in HMIS, this would be an error. Please double check with your HMIS System Administrator if you have any questions.

Exit Destination

A0
