Joplin Housing Connect Workflow

This article is designed to help guide HMIS users through the basics of entering clients into the Joplin Housing Connect CE project using the Client module of Wellsky Community Services. For additional assistance beyond this workflow article, please contact the ICA Missouri Helpdesk.

Client Entry at Initial Intake

Select appropriate Enter Data As provider (Housing Connect).

Client Search

Before entering any information into HMIS, projects must discuss and complete the HMIS Client Informed Consent to Share and Release of Information with each Head of Household. If the client does not sign the document, the user must first contact the ICA Helpdesk before doing anything in HMIS on behalf of the client.

  1. Enter Head of Household’s Name, Name data quality, SSN, SSN data quality and U.S. Military Veteran? Optional: Enter nickname in Alias.
  2. Search
    1. If a match is found, click green plus next to the name to open the client record.
    2. If no matches are found, verify name & SSN, click Add New Client with This Info.
  3. Back Date Prompt: change the date to intake date and click “Set New Back Date."

Client Profile Tab

  1. Enter or update missing/incorrect information in Client Record and Client Demographic.
  2. Add or update Client Profile Additional Information.
    1. Residence History sub-assessment
    2. Contact Information sub-assessment
    3. Emergency Contact sub-assessment
  3. Attach appropriate scanned documents in File Attachment.

Household Tab

(If there is only one person in the household, skip to ROI tab)

  1. If no household exists, click Start New Household.
    1. Answer Household Type question.
    2. Search and add all family members to household.
    3. Click Continue.
    4. Complete the head of household status and relationships.
    5. Complete the demographic information.
    6. After last family member, click Save & Exit.

ROI Tab

  1. Click Add Release of Information
    1. Fill in Release Granted.
    2. Fill in End Date with date 1 year from date signed.
    3. Select Documentation type.
    4. Click Save Release of Information.
    5. Click Exit.

Entry/Exit Tab

  1. Click Add Entry/Exit.
  2. Complete Entry Data.
    1. Select Type: Housing Connect selects Basic
    2. Confirm entry date is the date of intake at 12:00:00 AM
  3. Click Save & Continue.
  4. Complete the Joplin Housing Connect Assessment for all members of the household.
  5. Select the Head of Household.
  6. Toggle over to the Special Needs Assessment and complete for all members of the household.
  7. After last family member, click Save & Exit.

Referral to HMIS Provider

Home Page Dashboard

Select appropriate Enter Data As provider.

Clients Module

Client Search

  1. Click Add Referrals.
    1. Remember to check the boxes for the correct household members (the system automatically only selects the Head of Household).
    2. Select Supportive Housing Placement/Referral in the Service Code Quicklist; click Add Terms.
    3. Select the provider from the Referral Provider Quicklist dropdown; click Add Provider.
    4. In the Referrals section, check the box under Supportive Housing Placement/Referral.
    5. In the Selected Needs section, set Need Status to Identified and Service Pending.
    6. Click Save All.
  2. Click Exit.
