Last Date of Contact Report for Street Outreach and Supportive Services Projects and Coordinated Entry Projects

Street outreach and coordinated entry projects are expected to record a Current Living Situation in HMIS anytime there is an interaction between a worker and a client designed to engage the client. This report can be used to assist in reviewing whether Current Living Situations records have been properly recorded for clients being served by Street Outreach, Supportive Services, and Coordinated Entry projects.

Setting the Prompts

Prompt Instructions
EDA Provider If only one project is selected in the "Select Provider(s):" prompt, select the same project here.
If more than one project is selected in the "Select Project(s):" then select one of the projects within the selected projects or reporting group.
Select Provider(s): Select the project(s) for which you wish to run the report. 

Using the Report

This report has three tabs, each serving a specific purpose. 

Summary Tab

Shows the number of clients who have no CLS recorded in the last 90 days, per project that the report was run for. This report only shows clients who are either the head of household or an adult or if the date of birth is not entered.

Entry Exit-Based Detail Tab

Clients in bold, italicized, and red text indicate the client does not have a Current Living Situation (CLS) recorded within the last 90 days. The second to last column shows the last information date in a CLS (or that there is no CLS recorded). These clients must either be discharged from the project with an exit date that matches their last CLS date or has missing CLS records added to indicate that the client is still working with the project.

The requirement to discharge clients who have not had a CLS update within 90 days is set by HUD. 

CLS-Based Detail Tab

Shows information from the client's most recent recorded CLS. Clients in bold, italicized, and red text indicate the client does not have a Current Living Situation (CLS) recorded within the last 90 days. The second to last column shows the last information date in a CLS (or that there is no CLS recorded). These clients must either be discharged from the project with an exit date that matches their last CLS date or must have missing CLS records added to indicate that the client is still working with the project.
