HOPWA Data Collection Requirements

In addition to the HMIS Core Data Elements, HOPWA providers also are required to collect and enter the following data elements.

Recording HOPWA Service Transactions

  1. Make sure you've set your Enter Data As and your back date. The client's entry into the project should have been recorded in the HMIS prior to recording any service transactions. 
  2. Within the client record, click on Service Transactions.
  3. On the Service Transaction Dashboard, click Add Service.
  4. Select appropriate household members:
    1. If the service benefits the entire household, you can select the entire household or only the head of household. 
    2. If the service benefits a specific household member (other than the head of household), you will need to switch to that household member's file to record the service. 
  5. Ensure that the correct Service Provider is displayed and that your Start Date and End Date are correct. The start and end date fields should match the day the client was approved for this particular financial assistance. 
  6. Select the correct Service Type from the dropdown menu. Refer to the "HMIS Service Description" columns above to determine the appropriate type of service to select. 
  7. Click Save & Continue.
  8. Complete the HOPWA-specific fields:
    1. If recording a general service, select the appropriate service in the Type of HOPWA Service dropdown. Refer to the "HOPWA Services Crosswalk" above to determine the appropriate selection. 
    2. If recording financial assistance, select the appropriate financial assistance type in the HOPWA Financial Assistance Type and enter the amount in the HOPWA Financial Assistance Amount. Refer to the "HOPWA Financial Assistance Crosswalk" above to determine the appropriate selection. 
  9. There is no need to record the fund source used for HOPWA-related financial assistance. 
  10. Scroll down to Need Information and update the Need Status and Outcome of Need according to the client's situation.
  11. Click Save & Exit
