Assigning and Closing Case Managers in HMIS

Assigning Yourself as the Case Manager

Reminder: Client/Household intake must be entered into the HMIS project before assigning a case manager.

  1. Select appropriate Enter Data As provider
  2. Search for the client and select the record
  3. Set the appropriate back date
  4. Click on the Case Managers tab
  5. Click the Add Case Manager button
    1. If a household exists, select all family members which will be assigned to a case manager
  6. Set Type to Me
  7. Select Provider by making a selection from the drop down menu
  8. Type in the Start Date
  9. Click the Add Case Manager

*Clients can be assigned to more than one case manager at a time.

Closing Case Managers

Reminder: Case Managers should be closed when the relationship has ended. This could be when the client has been exited from the project and/or when the client is no longer contacting the Case Manager

  1. Select appropriate Enter Data As provider
  2. Search for the client and select the record
  3. Set the appropriate back date
  4. Click on the Case Managers tab
  5. Click on the pencil in front of the Case Manager's name to be closed
  6. Type in the End Date
  7. Click the Save Case Manager
