MoHIP Coordinated Entry Report - Balance of State

This report is to be run by each MoHIP funded Coordinated Entry project in the Balance of State CoC and submitted to MHDC each billing cycle.  Agencies must run the MoHIP Coordinated Entry report specific to your CoC.

Setting the Prompts

Prompt Instructions
EDA Provider Select the project for which you wish to run the report.
Start Date: Enter the start date for the reporting period in m/d/yyyy format. For example, if you're running a report for 7/1/2024-6/30/2025, the date entered in this box would be 7/1/2024.
Enter effective date Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2024-6/30/2025, the date entered in this box would be 7/1/2025.
Select Provider Adding Clients to PL: Select the project for which you wish to run the report. 
Select all Counties in MoHIP Region (optional): Some BoS CES providers operate in regions other than the ones for which they receive MOHIP funding. If your agency assesses clients only in the single region for which you receive MoHIP CE funding, skip this prompt.
If your agency assesses clients in multiple regions and you need to filter this report to pull in information only about clients assessed in the MoHIP region where you receive funding, then use this prompt to select all counties in that region

Using the Report

This report has three tabs, each serving a specific purpose.

Summary

This tab reports the number of clients assessed and placed on the prioritization list during the reporting period, as well as information about referrals to housing made by the selected provider during the reporting period.

Metrics are calculated as follows.

Metric How it's calculated
New Clients Assessed for Prevention and Diversion Clients who completed a Prevention/Diversion assessment during the report period where the Provider Completing Prevention/Diversion Assessment matches the provider selected in the "Select Provider Adding Client to PL" prompt.
Clients Successfully Diverted Clients who completed a Prevention/Diversion assessment during the report period where the Provider Completing Prevention/Diversion Assessment matches the provider selected in the "Select Provider Adding Client to PL" prompt and the client either was never added to the PL (has no PL Start Date) or they were on the PL at some point prior to the Prevention/Diversion assessment or if the client's latest Exit Outcome from the PL was Housed and the client self-resolved.
Clients Assessed for CE (completing VI-SPDAT) Clients who had a VISPDAT completed during the report period (based on the the Date of VI-SPDAT Assessment typed in the entry) and the Entry Exit Provider matches the provider selected in the "Select Provider Adding Client to PL" prompt.
New Clients Placed on PL Clients with a PL Start Date during the report period where the Provider adding client to the Prioritization List matches the provider selected in the "Select Provider Adding Client to PL" prompt.
All Clients on Active List During Reporting Period Clients with an active PL enrollment during the report period, based on PL Start Date and PL End Date where the Provider adding client to the Prioritization List matches the provider selected in the "Select Provider Adding Client to PL" prompt.
Housing Referrals Clients for whom a referral for housing was made during the report period by the provider selected in the "Select Provider Adding Client to PL" prompt.

Client Detail

This tab lists the IDs of clients that fall into each of the following categories: New Clients Assessed for Crisis Needs Assessments, Clients Assessed for CE, New Clients Placed on PL, and All Clients on Active List During Reporting Period.

Additional Information

This tab contains information about the report that can be utilized by ICA staff to determine whether the report was run properly by the user. If you seek assistance with this report from the ICA Missouri Helpdesk, please be prepared to read/send information off of this tab to the individual who assists you.
