ESG Case Management Services Provided Report
The ESG Case Management Services Provided Report provides a list of client demographics, services, and case notes documented within those service transactions for selected provider(s) within a designated time period. The report can be run to include all clients or can be run to include specific clients.

If not all service transaction codes need to be reported, use the View feature in order to utilize input controls on the report to select only the service codes needed.
Setting the Prompts
Prompt | Instructions |
EDA Provider | If only one project is selected in the "Select Service Provider:" prompt, select the same project here. If more than one project is selected in the Select Service Provider:" prompt, then select one of the projects. |
Enter effective date | Enter the date after the last date of the reporting period in M/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021. |
Report Start Date: | Enter the start date for the reporting period in m/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2020. |
Report End Date plus one Day: | Enter the date after the last date of the reporting period in m/d/yyyy format. For example, if you're running a report for 7/1/2020-6/30/2021, the date entered in this box would be 7/1/2021. |
Select Service Provider: | Select the project(s) for which you wish to run the report. |
Enter value(s) for Client Uid: | If pulling the report for all clients served by the project during the reporting period, leave this prompt blank. If pulling a report of all services provided to specific clients during the reporting period, enter the client ID for each individual who should be included in the report. |
Using the Report
The report generates a spreadsheet with information about clients with service transactions recorded during the selected report period. The top of the report will list the selected project(s). Each client ID will appear as its own table. Within each client ID table, a row of service specific information will appear. Information reported with each row will include the client HMIS ID number, the client name, age, gender, relationship to Head of Household, the project providing the service, the AIRS service code and description, the date of service, and any service notes.
Important! Because the report contains identifying information (names), the report cannot be emailed (even within agencies) unless encrypted, nor can it be shared outside of the agency.

Do not attach the report to any emails going to the helpdesk as the report contains personally identifying information and cannot be transmitted unless encrypted.
