MHDC Data Collection Forms
ICA has developed these data collection forms to assist with the collection of data that is entered into HMIS by Missouri Housing Trust Fund (MHTF) projects. The forms are laid out in manner that they are consistent with the data entry workflow within the HMIS. All forms are available in both Word Document (docx) format and PDF format. Use of these forms is optional.
