Housing Planning Case Management for St. Louis City DHS

In accordance with requirements of the St. Louis City Department of Human Services, all projects receiving one or more of the following funding sources must submit a Housing Planning Case Management report, either through the HMIS or through their own internal database. 

  • American Rescue Plan Act (ARPA) through City DHS
  • Continuum of Care (CoC) - City DHS Subrecipients Only
  • Emergency Solutions Grant (ESG & ESG-CV) - City DHS Subrecipients Only
  • City Prop P Funding
  • State Domestic Violence Funding through DHS

One of the requirements of the Programmatic Evaluation Report Requirements is that specific projects must submit the Housing Planning Case Managment Report, either from HMIS, or a comparable report from an internal system. The report, whether from HMIS or an internal database, must show that the agency has either achieved or is working toward achieving the following goals: 

  • 100% of clients are engaged on a monthly basis
  • 80% or more of clients are working on an active and documented housing plan, which may include topics such as:
    • Mental Health Services
    • Substance Use Services
    • Medical/Physical Health Services
    • Employment Services
    • Entitlement Linkages (SOAR/SSI/SSDI, SNAP, Medicaid, etc.)
    • Appropriate Documentation
    • Current Housing Plan Type
