Recording Housing Planning Case Management Activities for St. Louis City DHS in HMIS
This guide explains how to properly record housing planning case management activities in HMIS for reporting to St. Louis City DHS. This is an optional feature which agencies can request be enabled by project if the agency wishes to record this information in HMIS (instead of an agency's own internal database). Need-to-Know
In order to meet reporting requirements, agencies must record one record per head of household per month, with the note date matching a date a representative of the agency spoke to the client about the applicable topics. Agencies are permitted, if they so choose, to record more than one record per month, but only one record is expected per month for reporting purposes.
Recording a Note
To record a Housing Case Management note, find the Housing Case Management sub-assessment table, like the one shown below. In general, this can be found under the "Assessments" tab in the Clients module or as a tab within the entry/exit area.

- Click "Add" to create a new record. A pop-up will appear, as shown below.

- Ensure the Note Date matches a date your project met with the client.

- Fill in the "Provider completing case notes" with your agency's name. If your agency name does not appear in the dropdown, contact the helpdesk at mohmis@icalliances.org for assistance.

- For each of the service types, select the option that best matches the current situation as of the date you or another agency representative spoke to the client. For the purpose of the report, all options are equally valid - selecting "Worker unable to determine" will not cause any errors or reduced points on your report.

- If you are providing the client with any other services or referring them to other services, please list them here. If no other services are being provided or referred to, this may be left blank.

- Select the closest option for the "Current Housing Plan" that reflects the client's current housing plan. It is understood that this may change or that they may have multiple plans, and that's okay. Just select the closest available option for their most preferred plan at this time.

- If you wish to do so, you may record case notes here. This is completely optional and will not affect the information in the report generated by BusinessObjects.

- Click "Save".
